A professional assessment is helpful when the elder has experienced changes in health or major life events. These are moments when options seem either limited or overwhelming and it’s unclear what to do. The elder and their family may also feel that their goals are being threatened. An assessment gathers information about all aspects of life: social, psychological, physical and cognitive health, functional ability, spiritual, safety, availability of support, financial resources, and legal issues. These areas are reviewed in light of the elders goals and wishes. A thorough review provides direction on what needs to be addressed in the short term and long term to honor those wishes. The outcome of an assessment are recommendations on how to address issues revealed and achieve established goals. At the end of an assessment, the family and/or elder should have concrete recommendations to think about, react to and implement.
